I am in network with several major insurance companies including Motor Vehicle Accidents and Workers Compensation. I am out of network with UHC, Aetna and CHP. Superbills can be provided for out-of-network billing in some cases.

Coverage and benefits will be confirmed when you call to schedule.

If you do not have insurance, time of service rates are available.

If you are a new patient interested in booking, please call the clinic to schedule 503-445-7999. You will need to fill out paperwork prior to your first appointment. If you are planning to use Insurance, please have that information available.
